Liberation-Based Therapy LCSW, PLLC is a psychotherapeutic group practice focused on supporting mental health and wellness through the lens of liberation-based healing practices. The practice serves everyone with a focus on BIPOC, immigrant communities / stories, LGBTQIA2S+ and intimate partnerships, including integrated families and open relationship structures. The Referral Lead will primarily support the BTH program day-to-day but will also support larger COR initiatives such as Special Medical Needs Shelters (SMNS) and Hotel Quarantine Isolation Program (HQIP) and other projects as they arise. The Referral Lead will report to the Executive Director of COR. The referral lead will report to the Senior Assistant Vice President of COR and have a dotted line to the Office of Behavioral Health Complex Needs team for clinical support.
• Develop standard workflow for reviewing and accessing incoming referrals to the BTH program
• Interview potential guest for the program to see if they are interested and suitable for the program
• Work closely with the BTH local Site Directors to understand the differences in site capabilities in order to better triage referrals to sites
• Liaise with referring staff from NYC Health + Hospitals facilities including: Inpatient Psych, Expended Care Unit, Correctional Health, Safety-net and PORT clinics to develop a continuous pipeline of referred patients and actively maintain waiting list
• Run regular in-service trainings with BTH Site Directors and Psychiatrists to help facility staff better understand the program and what will make a successful candidate
• Support Epic referral enhancements as needed
• Work with the COR team, to build out preparedness models to support future citywide or tactical activations, including, but not limited to Special Medical Needs Shelters and Hotel Quarantine Isolation Program
• Support coordination as needed with hospitals, long-term care facilities, ambulatory sites, shelters of all kinds, and community partners during emergencies to ensure continuity of care

The Legal Aid Society’s Civil Practice has an opening for a Forensic Social Worker within the Elder Law Unit. The Elder Law Unit provides legal representation and support to seniors in housing court at risk of eviction. Elder Law Unit clients often present with substantial needs inextricably linked with the core eviction defense case. These needs/situations most often include (1) cognitive limitations or decline; (2) physical and/or mental health related disabilities, (3) Elder Abuse (physical, financial, emotional); (4) inability to continue to live in the community without additional support (medical, financial etc); (4) are not receiving all of the benefits they could be eligible for; and (5) other issues stemming from one or more of the above. Addressing these complex needs are integral to maintaining a client centered focus in the representation and to allow the client to maintain autonomy and to live in the community safely as long as possible. The Legal Aid Society, the largest social justice public interest law firm in New York City, has an immediate opening for a dedicated and passionate court-based Forensic Social Worker in the Parole Revocation Defense Unit (PRDU) in the Bronx office to provide client services. While working closely with the Criminal Defense attorneys, the Forensic Social Worker’s (FSW) primary role is to interview clients, and any collateral contacts, with the goal of gathering a life history to highlight mitigating circumstances such as mental illness, drug addiction, trauma and/or chronic poverty leading to a client’s involvement in the criminal justice system.
The FSW will then advocate for an alternative to detention, an alternative to incarceration, or lesser sentence, based on mitigating factors. The FSW will evaluate clients and recommend a treatment program. The FSW will fill out necessary paperwork and provide supporting documents to facilitate a client’s interview with a program and/or to assure a client’s acceptance into a program. The FSW educates the client about treatment options, offers crisis intervention, and short-term counseling. The FSW provides essential information to attorneys about clients.
The Parole Revocation Defense Unit, through a dedicated team of lawyers, paralegals, investigators, and social workers, represents over 6000 parolees per year with the stated goal of both shortening their length of incarcerations on Rikers Island, and upon release, assisting parole clients in the community to avoid violations and re-incarceration.

The licensed social worker (LMSW) provides frontline emergency response in the community, supporting children to older adults experiencing mental health crises, substance use challenges, suicidality, trauma, and acute stress. Core responsibilities include de-escalation, brief crisis interventions, comprehensive risk assessments, and rapid connection to appropriate services. The duties are primarily performed in field-based settings.
