SAGE is the world’s largest and oldest organization dedicated to improving the lives of lesbian, gay, bisexual, transgender, and queer or questioning (LGBTQ+) older adults. Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources for LGBTQ+ older adults and their caregivers, advocates for public policy changes that address the needs of LGBTQ+ older people, and provides cultural competency training for aging providers and LGBTQ+ organizations, largely through its National Resource Center on LGBTQ+ Aging and its growing social enterprise, SAGECare.
The Director of Care Management provides overall leadership and accountability for SAGE’s Care Management programming across New York City. This role is responsible for contract management, program development and oversight, and the supervision of departmental staff. The Director directly supervises multiple team members and offers strategic leadership, guidance, and operational direction for all Care Management services. In collaboration with internal SAGE departments and external partners, the Director helps deliver on-site programs and activities that advance health, wellness, and a strong, vibrant community for LGBTQ+ older adults. This role will report to the Senior Director of SAGEServes Supportive Services.

The Director brings a solid background in care management and social service programming, with the ability to oversee complex, citywide operations while maintaining a person-centered, affirming, and strengths-based approach.
This individual is an effective and experienced people manager who can mentor, develop, and hold multiple staff accountable while fostering an inclusive, collaborative, and high-performing team culture. They are highly organized and detail-oriented, with a strong understanding of regulatory and contractual compliance, and the ability to manage multiple priorities, deadlines, and stakeholders simultaneously.
The ideal candidate is also a strategic thinker who can contribute to the growth and evolution of Care Management programming while remaining engaged in day-to-day problem solving and operational decision-making. They demonstrate sound judgment, strong communication skills, and the ability to build and sustain relationships with LGBTQ+ older adults, colleagues, community partners, and public and private funders.
