Job Description

Community League of the Heights (CLOTH) seeks to hire a Social Worker who will be responsible for assessment, case management, referrals, and counseling for participants on-site for the Rangel PACT Social Services Program. This role focuses exclusively on supporting residents’ emotional and psychological well-being through counseling, mental health screenings, crisis intervention, and emotionally informed care.


The Social worker will be primarily based on site as a part of a multidisciplinary PACT social services team serving a public housing community.


The Social Worker will work closely with Rangel Resident Coordinators, who will identify residents in need of mental health services and refer them directly for care.

Job Responsibilities

  • Provide individual counseling using trauma-informed, strengths-based, and culturally competent approaches
  • Conduct mental health screenings and psychosocial assessments
  • Deliver crisis intervention and short-term stabilization
  • Serve as primary mental health contact for referred residents
  • Facilitate group counseling sessions and mental health workshops
  • Collaborate with Resident Coordinators and interdisciplinary teams
  • Develop individualized care plans and provide referrals as needed
  • Maintain confidential clinical documentation in compliance with HIPAA
  • Provide reporting to Supervisor on a regular basis

Job Qualifications

Skills, Behaviors and Experience Required


  • Master’s degree in Social Work (MSW)
  • 2–3 years of post-graduate clinical experience
  • Experience with counseling, crisis intervention, and groups
  • Strong cultural competence and communication skills
  • Effective in working with individuals, families, and groups.
  • Experience with working with children, youth, adults, and senior citizens
  • Ability and experience in working in a fast-paced team-oriented environment.
  • Proficient in all Micro Soft Office Applications: PPT, Word, Excel
  • Excellent presentation skills
  • Strong oral and written communication skills
  • Excellent problem solver
  • Ability to work closely with people of diverse backgrounds, ages, and cultures.


Preferred Qualifications

  • Experience in public housing, PACT, or community mental health
  • Active NY State Social Work license (LMSW)
  • Bilingual Spanish/English
  • Knowledge of NYC mental health systems
